Why I made it

I look after my mom. I'm not a nurse or a doctor. I'm her son.

When I take her in, I bring a printout of all her meds, doctors, conditions and allergies. It has made every doctor happy, every time. Some have shown it around the office and wished more patients and caregivers would do the same.

So I cleaned mine up to share.

Good questions

What should I bring to a parent's doctor appointment?

A current list of their medications with doses, their doctors, conditions and allergies, plus your questions written down. The Care Sheet puts all of that on a few printed pages you can hand over.

What makes this different from a blank medication list?

It has a page that says what is normal for the person: how their memory, walking and appetite usually are. A doctor who just met them can't know that, and it helps show what has changed. There is also a page for each visit, for your questions and what the doctor says.

Where does my information go?

Nowhere. It's a PDF on your own device, and I never see what you type. Don't put a Social Security number on it, and only share it with people you trust.

Can I fill it out by hand?

Yes. Type into it in a PDF app, or print it blank and use a pen.

Are you a doctor or a nurse?

No. I'm a son who looks after his mom. This is an organizing tool, not medical advice, so check it against the bottles and with your pharmacist.
